I wish to become a member of the PRIME Network.

I confirm that:

• I have read the Foundation Principles of PRIME - Partnerships in International Medical Education and undersood that these form the principles and basis of the PRIME Network.
• I wish to both practise and teach in a manner that accords with these principles.
• I share the vision of working together to establish better health care worldwide based on the whole person approach of body, mind and spirit that PRIME promotes.
• I understand that PRIME is a not-for-profit organisation. I will not seek any financial gain from my membership of the PRIME Network and agree that materials published by PRIME may not be used for commercial purposes.

Data Protection: By compleing this form I consent to PRIME recording and storing this information in accordance with the Data Protection Act 1998 and the PRIME Data Protection Statement, available on request. I understand that no details will be released outside PRIME without my express permission. I agree to notify PRIME of any significant changes to this information and/or if I wish to be removed from the PRIME Network at any time.
